- Vendor type. (Food Concession Booth, Commercial Booth or Non-Profit)
- Vendor Application
- W-9
- Certificate of Liability Insurance
- Submit application by email
- Payment must be received
- Food Handler Permit
- All non food trucks must contact local health department for temporary Sales Permit by calling 435-623-0696
All Vendor paperwork and payment DUE July 15th, 2025 by 5:00 PM
